Sled Hockey Waivers and Forms

Fair Play Committee Procedures
-
Between September 1st and December 31st, the committee will review sled hockey waiver requests.

The Committee will not review waivers submitted for player/program grievances.

If a player wishes to leave their current program and join another, they must go through their local USA Hockey Affiliates dispute management process.

USA Hockey Sled Classic (Fall)

The New York Islanders will hold the 2025 USA Hockey Sled Classic from December 4th through 7th at the Dix Hills Ice Rink and The Rinx in Hauppauge, NY.

USA Hockey Sled National Championships (Spring)

The 2026 USA Hockey Sled National Championship will be held from April 20th through May 3rd at the Children's Health StarCenter in Dallas, Texas.

The Championship includes each level of sport, including youth.

USA Sled Hockey Camp

The USA Hockey Sled Player Development Camp takes place annually in July.

Players are evaluated for potential future selection to the U.S. Women’s Development Sled Hockey Team, U.S. National Sled Hockey Team, or the U.S. National Sled Hockey Team practice squad.

The selection process is invitation-only.

Male and female players who are over the age of 15 on June 30th, and are nominated by their coach, are eligible to be selected for an invitation.

Nominations are due April 15.

Regional Sled Hockey Player Development Camps

USA Hockey offers regional camps at several locations around the country.
-
Camps are open to all players with at least one year of experience in sled hockey.

The U.S. National Sled hockey team players and coaching staff assist in coaching each camp!
